Summary

Heart period variability, a measure of parasympathetic activity, is lower in heart failure patients post-transplant. It improves significantly 3-6 months after cardiac transplantation, indicating recovery of autonomic function.

Background:

  • Congestive heart failure (CHF) is associated with impaired autonomic function.
  • Orthotopic cardiac transplantation aims to restore cardiac function, but the recovery of autonomic control, particularly parasympathetic activity, requires investigation.

Purpose of the Study:

  • To assess baseline parasympathetic activity using heart period variability in patients with CHF before and after orthotopic cardiac transplantation.
  • To compare parasympathetic activity in transplanted patients with age-matched controls.
  • To investigate the temporal changes and influencing factors of parasympathetic activity post-transplantation.

Main Methods:

  • Heart period variability (standard deviation of 120 consecutive RR or PP intervals) was measured in 18 CHF patients pre- and post-transplant, and 16 controls.

    • Significant improvement in parasympathetic activity occurs between 3 and 6 months post-transplantation.
    • The donor heart does not appear to undergo vagal reinnervation, suggesting recovery is primarily from recipient autonomic pathways.
