Background:

Erectile dysfunction (ED) is increasingly recognized as a potential early indicator of systemic vascular disease.
The narrow penile artery may manifest atherosclerosis before other vascular conditions become apparent.
Underreporting of ED is a significant concern due to its implications for broader cardiovascular health.

Purpose of the Study:

To investigate the prevalence of ED in relation to age.
To examine the management and treatment patterns of ED in primary care settings.
To assess the correlation between ED severity, age, and discussion with healthcare providers.

Main Methods:

A questionnaire on ED prevalence and management was administered to men in primary care waiting rooms.
Data analysis focused on the relationship between ED and age, including prevalence, severity, discussion, and treatment.
A high response rate of over 95% was achieved.

Main Results:

ED prevalence was 77% in men aged 70+ compared to 61% in men aged 40-69 (p = .0001).
ED showed a strong linear correlation with age (R(2) = .80, p < .0001).
Over half of men with ED had not discussed it with a provider; 72% with a history of ED were never treated. Younger men were more likely to be treated than older men.

Conclusions:

ED prevalence significantly increases with age and is strongly correlated with it.
A substantial number of men with ED, particularly older men, do not seek or receive treatment, indicating a gap in care.
Given ED's role as a vascular marker, improved assessment and follow-up models are warranted to address underreporting and ensure timely intervention.
