Summary
Management of brainstem cavernous malformations (CMs) in pregnant patients is rare. Surgical intervention during pregnancy is typically reserved for life-threatening cases, with most patients treated post-delivery.

- Pregnant patients with eloquent brainstem CMs require specialized multidisciplinary care to determine the optimal timing for intervention.
