Background:

Palliative care services are increasingly adopted in hospitals, aiming to reduce costs and manage symptoms.
However, there's a lack of systematic guidelines for triggering palliative care consultations, leading to underutilization and misunderstanding of these services.

Purpose of the Study:

To pilot test a novel palliative care screening tool designed to guide clinician judgment.
To assess the tool's effectiveness in identifying appropriate candidates for palliative care consultation.

Main Methods:

A one-page, face-valid screening instrument was developed based on expert opinion.
The tool was piloted to assess its utility in guiding palliative care referrals.

Main Results:

The study included 74 patients (55.4% female, average age 63.4 years) with an average length of stay of 22.7 days.
Pain was the most prevalent moderate-to-severe symptom (31% at entry, 20% at discharge), with significant overall reduction (p < 0.0001).
Implementing the screening tool increased palliative care referrals compared to the previous year, despite an initial threshold potentially being too high.

Conclusions:

The developed screening tool demonstrated an increase in palliative care consultations.
The pilot suggests the tool can aid in identifying patients who may benefit from palliative care services, thereby improving access and utilization.
