Summary

Transcranial magnetic stimulation (TMS) accurately predicts hand function after hemispherotomy in children with polymicrogyria. This noninvasive method helps assess sensorimotor function before epilepsy surgery.

Purpose of the Study:

  • To evaluate the predictive value of transcranial magnetic stimulation (TMS) and functional magnetic resonance imaging (fMRI) for postoperative hand function in boys undergoing hemispherotomy for hemispheric polymicrogyria.
  • To compare the utility of TMS and fMRI in assessing the sensorimotor system in this patient population.

Main Methods:

  • Case series of four boys with hemispheric polymicrogyria, pharmacoresistent epilepsy, and hemiparesis.

  • This fMRI-observed activation was not essential for maintaining grasp function in the paretic hand.

Conclusions:

  • TMS is a reliable noninvasive tool for predicting preserved hand function after hemispherotomy in pediatric patients with hemispheric polymicrogyria.
  • Sensorimotor fMRI findings in patients with congenital hemiparesis require careful interpretation, as ipsilesional activation may not directly correlate with motor function preservation.
  • Preoperative TMS can guide surgical decisions and patient counseling regarding functional outcomes following hemispherotomy.
