Frequent premature atrial contractions can cause symptomatic bradycardia. This case study shows successful treatment of bigeminal premature atrial contractions with propafenone, managing bradycardia effectively.
Area of Science:
- Cardiology
- Electrophysiology
- Pharmacology
Background:
- Premature atrial contractions (PACs) are common arrhythmias.
- PACs can sometimes lead to bradycardia through sinoatrial node modulation.
- Bigeminal patterns of PACs can exacerbate bradycardic effects.
Observation:
- A patient presented with symptomatic bradycardia.
- The bradycardia was associated with frequent premature atrial contractions in a bigeminal pattern.
- Sinoatrial node resetting by PACs was suspected as the underlying mechanism.
Findings:
- The patient's symptomatic bradycardia was successfully managed.
- Propafenone was administered as the treatment of choice.
- The antiarrhythmic drug effectively controlled the premature atrial contractions and resolved the bradycardia.
Implications:
- This case highlights propafenone as a viable treatment option for PAC-induced bradycardia.
- Understanding the mechanism of PACs on sinoatrial node function is crucial for management.
- Effective treatment of PACs can alleviate symptoms and prevent complications associated with bradycardia.
