Free perforator flaps offer the highest profit in breast reconstruction surgery under Germany's Diagnosis-Related Groups (DRG) system. However, the DRG system inadequately reflects current medical standards and inflation, posing economic risks.

Area of Science:

  • Medical economics
  • Surgical oncology
  • Health services research

Context:

  • The German Diagnosis-Related Groups (DRG) system, implemented in 2004, classifies hospital cases for medico-economic purposes.
  • Breast reconstruction surgery following cancer treatment is a significant area within this classification.

Purpose:

  • To analyze changes in the fee-per-case system for breast reconstruction over the past six years.
  • To evaluate the economic outcomes (length of stay, reimbursement, costs, profits) of various reconstructive methods including immediate, delayed, pedicle, and free flaps, as well as alloplastic techniques.

Summary:

  • Free perforator flaps yielded the highest profit margins, up to three times greater than alloplastic and pedicle flaps.
  • The analysis accounted for inflation, revealing potential foregone profits due to the retrospective nature of cost calculations.

  • Current DRG reimbursement inadequately accounts for immediate and bilateral breast reconstructions, despite advancements in medical guidelines.
  • Impact:

    • The study highlights a disconnect between evolving medical standards in breast reconstruction and the DRG system's economic mapping.
    • Increasing reliance on market mechanisms risks economic selection overriding clinical recommendations.
    • The German DRG system demonstrates a lack of adaptability to new medical advancements, even years after its introduction.
