Background:

  • Extended criteria donor (ECD) kidneys may lack sufficient renal mass for single transplantation.
  • Dual kidney transplantation (DTx) is proposed to maximize the utility of ECD kidneys.

Purpose of the Study:

  • To compare the outcomes of single kidney transplantation (STx) versus dual kidney transplantation (DTx) using ECD kidneys.
  • To evaluate the impact of donor and graft characteristics on transplant success.

Main Methods:

  • A case-control study analyzed 80 kidney transplants from donors over 65 years old between May 2007 and March 2011.
  • Transplant criteria included biopsy score, donor age, medical history, kidney size, and creatinine clearance.
  • 40 kidneys were used for STx and 40 for DTx.

Main Results:

  • DTx donors were older and had a higher proportion of female donors compared to STx.
  • Kidneys for DTx had higher biopsy scores; recipients were older.
  • No significant differences in glomerular filtration rate or proteinuria were observed between groups.
  • DTx recipients showed better creatinine clearance at 6 months (P<.05).
  • Three-year graft survival was 90% for both STx and DTx.

Conclusions:

  • DTx demonstrates favorable survival and renal function outcomes, facilitating the use of ECD kidneys.
  • Decision-making for DTx should integrate pre-transplant histological assessment and donor clinical factors.
  • DTx offers a viable strategy for utilizing organs potentially unsuitable for STx.
