Background:

  • Takayasu's arteritis is a rare, large vessel vasculitis affecting the aorta and its branches.
  • Cerebral involvement in Takayasu's arteritis can lead to severe neurological deficits.

Observation:

  • A 32-year-old female presented with syncope and hemiparesis due to Takayasu's arteritis.
  • Imaging revealed cerebral infarcts and significant stenosis/occlusion of major arteries supplying the brain and upper extremities.
  • The patient had rigid arterial lesions due to diffuse fibrosis.

Findings:

  • Medical management with prednisone and anti-platelet therapy stabilized the patient.
  • Single-stage multiple stenting successfully treated the severe arterial stenosis.

  • Despite improved hemodynamics, neurological deficits did not fully resolve.
  • Implications:

    • Single-stage multiple stenting is a viable first-line treatment for complex arterial lesions in Takayasu's arteritis.
    • Early intervention may improve outcomes in patients with Takayasu's arteritis and cerebral ischemia.
    • Further research is needed to optimize neurological recovery in these patients.
