Summary

A rare case of parathyroid carcinoma presenting as a mediastinal mass was successfully treated. Surgical removal of the parathyroid cancer normalized parathyroid hormone levels, indicating a positive outcome.

Background:

  • Primary hyperparathyroidism can rarely be caused by parathyroid carcinoma.
  • Parathyroid crisis is a severe manifestation requiring urgent intervention.

Observation:

  • A 72-year-old woman presented with hyperparathyroidism and a high parathyroid hormone (PTH) level (808 pg/mL).
  • Initial surgery revealed only two superior parathyroid glands, with one hypertrophied.
  • A subsequent CT scan identified a mediastinal mass, later confirmed as parathyroid carcinoma.

Findings:

  • Post-surgical PTH levels remained elevated after initial exploration (726.5 pg/mL), decreasing significantly (5.74 pg/mL) after mediastinal mass resection.
  • Cytofluorometric analysis showed diploidy in the remaining parathyroid glands but aneuploidy in the carcinoma.
