Background:

Mixed autoimmune hemolytic anemia (AIHA) involves both warm and cold autoantibodies.
Diagnosis requires specific serological findings, including IgG, C3d, and cold agglutinins.

Observation:

A rare case of primary mixed AIHA was observed in a 12-year-old girl.
The patient presented with symptoms indicative of AIHA.

Findings:

The patient's AIHA was characterized by the presence of both warm and cold autoantibodies.
Diagnostic tests confirmed IgG and C3d on red blood cells, along with cold agglutinins.

Implications:

This case highlights the importance of comprehensive diagnostic approaches for mixed AIHA.
Corticosteroid therapy can be effective in managing pediatric mixed AIHA.
Further research into rare pediatric autoimmune hematological disorders is warranted.
