Summary

A significant 13.4% of women required hysterectomy after endometrial ablation. Younger age at ablation and specific ablation types increased this risk, highlighting the need for careful patient selection and procedure choice.

Area of Science:

  • Gynecology
  • Minimally Invasive Gynecological Surgery

Background:

  • Endometrial ablation is a common treatment for abnormal uterine bleeding.
  • Hysterectomy remains a definitive treatment but involves significant morbidity.

Purpose of the Study:

  • To determine the incidence of hysterectomy following endometrial ablation.
  • To identify factors associated with subsequent hysterectomy.

Main Methods:

  • Retrospective cohort study including 1169 women undergoing endometrial ablation.
  • Minimum follow-up of 9 months; data collected from a gynecology practice.

Main Results:

  • 13.4% of women (157/1169) underwent hysterectomy post-ablation.
  • Younger age at ablation and prior cesarean delivery were associated with higher hysterectomy rates.

  • Hysterectomy rates and timing varied significantly by ablation type (rollerball, thermal balloon, radiofrequency, cryoablation).
  • Adenomyosis was found in 44.4% of hysterectomy specimens.
  • Conclusions:

    • 13.4% of women required hysterectomy within a mean follow-up of 39 months after endometrial ablation.
    • Patient age at ablation and the specific ablation technique are key factors influencing the need for subsequent hysterectomy.
