Intrachiasmal hematomas, often caused by underlying vascular malformations, can be successfully treated with surgery. Surgical evacuation of these hematomas frequently improves vision in patients with chiasmal syndrome.

Area of Science:

  • Neuroscience
  • Ophthalmology
  • Neurosurgery

Background:

  • Acute chiasmal syndrome can be caused by various lesions compressing the optic chiasm.
  • Intrachiasmal hematomas are a rare but significant cause of visual field defects.

Observation:

  • A 26-year-old patient presented with acute chiasmal syndrome.
  • Imaging revealed an intrachiasmal hematoma.
  • The hematoma was surgically evacuated via a right subfrontal approach.

Findings:

  • Histological examination confirmed an arteriovenous malformation as the underlying cause.
  • Review of 21 similar cases showed surgical intervention generally improved ophthalmological signs.
  • Subjacent lesions, including vascular malformations and gliomas, were identified in 75% of literature cases.

Related Experiment Videos

Implications:

  • Surgical evacuation is an effective treatment for intrachiasmal hematomas.
  • Identifying and treating underlying lesions is crucial for successful management.
  • This case highlights the importance of considering vascular malformations in the differential diagnosis of acute chiasmal syndrome.
