Transitioning from peritoneal dialysis to renal transplant: a diabetes management case study

Renal transplantation represents a challenge for diabetes management both for controlling blood glucose values while on intravenous steroids and for transitioning the patient to a new outpatient insulin regimen. M.B. is adjusting to a "new normal" with a functioning kidney and increased insulin requirements. A renal/diabetes care provider can have a significant impact on the recovery of patients after transplantation by spending additional time listening to patients' concerns, educating them on strategies to effectively manage hyperglycemia while avoiding hypoglycemia and building trust in the healthcare team and treatment plan.
