Small retinal vessel occlusion is a rare but severe complication of Takayasu arteritis (TA). This microcirculatory issue can be an initial sign of TA and significantly impacts visual prognosis.

Background:

  • Takayasu arteritis (TA) is a large vessel vasculitis primarily affecting the aorta and its branches.
  • Small vessel involvement, particularly in the retina, is less commonly described but can have significant consequences.

Observation:

  • This study analyzed 9 patients with Takayasu arteritis and small retinal vessel occlusion.
  • Retinal artery occlusion occurred in 7 patients, while 2 had venous involvement.
  • Small retinal vessel occlusion was the presenting symptom in 44.4% of cases.

Findings:

  • The majority of patients (55.6%) had Type V Takayasu arteritis, with 88.9% showing supra-aortic branch involvement.
  • While immunosuppressive therapy improved or stabilized the condition in some (6/9), 3/9 experienced worsening.
