Porcelain gallbladder, a condition with calcified gallbladder walls, was once a contraindication for laparoscopic cholecystectomy due to cancer risk. This study shows Type I porcelain gallbladder can be safely managed laparoscopically with careful patient selection.

Area of Science:

  • Gastroenterology and Surgical Oncology

Background:

  • Porcelain gallbladder (PG) is characterized by calcification of the gallbladder wall.
  • Historically, PG was considered a relative contraindication for laparoscopic cholecystectomy (LC) due to concerns about gallbladder cancer.
  • Preoperative ultrasound is crucial for classifying PG and assessing associated risks.

Observation:

  • Five cases of PG (0.04%) were identified among 12,000 cholecystectomies.
  • Three cases were classified as Type I (complete wall calcification) and two as Type II.
  • One case presented with associated gallbladder carcinoma.

Findings:

  • Laparoscopic cholecystectomy was performed in three cases of Type I PG.
  • The procedure presented challenges, including adhesions and difficulty grasping the thickened gallbladder wall.

Application of the En Bloc Concept Combined with Anatomic Resection in Laparoscopic Hepatectomy

Published on: March 10, 2023

  • Postoperative recovery was uneventful, with histopathology confirming Type I PG without malignancy.
  • Implications:

    • Laparoscopic cholecystectomy is a feasible option for selected Type I porcelain gallbladder cases.
    • Careful preoperative selection based on ultrasound findings is essential.
    • While conversion rates may be higher, LC offers potential benefits for these patients.
