Histology of the Uterus

The uterine wall consists of three histological layers: the perimetrium, myometrium, and endometrium. The outermost perimetrium is a thin, serous membrane connected with the broad ligament on the sides, which helps anchor the uterus in the pelvic cavity. The thickest layer, myometrium, is mainly made up of smooth muscle tissue bundles. Its contractions are vital in facilitating the expulsion of the uterine lining, fetus, and placenta during menstruation and childbirth.

Summary

A pregnant woman with a uterine fibroid developed severe pre-eclampsia and placenta accreta. Early diagnosis and management were crucial for the survival of both mother and preterm infant.

Background:

  • Submucosal uterine fibroids can complicate pregnancy.
  • Placental implantation on fibroids poses risks.
  • Pre-eclampsia is a serious pregnancy complication.

Observation:

  • A 35-year-old woman with a posterior wall fibroid conceived spontaneously.
  • Placental implantation occurred over the fibroid.
  • Severe pre-eclampsia with pulmonary edema developed at 32 weeks.

Findings:

  • Pregnancy terminated at 33+4 weeks via cesarean section due to severe pre-eclampsia.
  • Placenta accreta was diagnosed histopathologically, adherent to a pedunculated fibroid polyp.
  • The preterm infant survived.
