Elvitegravir/cobicistat/emtricitabine/tenofovir disoproxil fumarate (EVG/COBI/FTC/TDF) is a new, once-daily single-tablet regimen for treatment-naïve HIV patients. This investigational drug shows high efficacy and good tolerability, but requires further monitoring for renal function and resistance.
Area of Science:
- Pharmacology
- Virology
- Clinical Trials
Background:
- Elvitegravir/cobicistat/emtricitabine/tenofovir disoproxil fumarate (EVG/COBI/FTC/TDF) is an investigational once-daily co-formulation.
- It combines nucleotide/nucleoside reverse transcriptase inhibitors (FTC, TDF) with an HIV-1 integrase strand transfer inhibitor (EVG) and a pharmacokinetic booster (COBI).
Purpose of the Study:
- To review the investigational single-tablet regimen (STR) EVG/COBI/FTC/TDF.
- To compare EVG/COBI/FTC/TDF with other STRs, including EFV/FTC/TDF, RPV/FTC/TDF, and the investigational DTG/3TC/ABC pill.
Main Methods:
- Medline review of Phase II and III clinical trials.
- Review of abstracts from major HIV and infectious disease conferences (2010-2012) focusing on EVG/COBI/FTC/TDF.
Main Results:
- EVG/COBI/FTC/TDF offers a new, once-daily, single-tablet alternative for treatment-naïve HIV patients.
- The co-formulation demonstrates high efficacy and a favorable tolerability profile.
Conclusions:
- EVG/COBI/FTC/TDF represents a significant advancement in HIV treatment options.
- Further monitoring is necessary for renal function and virologic failure with resistance development.
- Single-tablet regimens are setting a higher standard for HIV treatment and drug development.
