Background:

  • Inflammatory bowel disease (IBD) diagnosis relies on integrating clinical, radiographic, endoscopic, surgical, and histologic data.
  • A subset of IBD patients (5-10%) present with indeterminate colitis (IC), lacking clear features of ulcerative colitis (UC) or Crohn's disease (CD).
  • Accurate classification of IC is crucial for appropriate patient management and treatment strategies.

Purpose of the Study:

  • To investigate the potential of microRNA (miRNA) biomarkers in differentiating classically diagnosed indeterminate colitis.
  • To compare miRNA expression profiles in IC with those of established ulcerative colitis and Crohn's disease.
  • To explore miRNA as a molecular tool for improving the classification of indeterminate colitis.

Main Methods:

  • Analysis of colonic mucosa samples from 53 patients: 16 IC, 14 CD, 12 UC, and 11 diverticular disease controls.
  • Total RNA extraction followed by quantitative reverse-transcription PCR using five specific miRNA primers (miR-19b, miR-23b, miR-106a, miR-191, miR-629).
  • Statistical analysis (ANOVA) to assess differences in miRNA expression among the patient groups.

Main Results:

  • Significant differences in miR-19b, miR-106a, and miR-629 expression were observed between UC and CD groups.
  • The average expression of all five miRNAs differed significantly between IC and CD groups.

  • Fifteen out of sixteen IC patients displayed miRNA expression patterns resembling UC, with only one showing a CD-like pattern.

Conclusions:

  • MicroRNA expression patterns in indeterminate colitis are predominantly similar to ulcerative colitis, not Crohn's disease.
  • These findings provide molecular evidence suggesting that most indeterminate colitis cases may represent a form of ulcerative colitis.
  • Further validation with long-term clinical outcome data is warranted, but miRNA analysis shows potential for enhanced IC classification.
