Resection arthroplasty effectively relieves pain after shoulder implant failure but results in poor function. Outcomes are worse following reverse shoulder arthroplasty compared to total or hemiarthroplasty.

Area of Science:

  • Orthopedic surgery
  • Shoulder reconstruction
  • Arthroplasty outcomes

Background:

  • Increasing rates of shoulder arthroplasty lead to more revision procedures.
  • Resection arthroplasty is an option when revision is not feasible.
  • Limited comparative data exist for resection following different primary shoulder arthroplasties.

Purpose of the Study:

  • To compare outcomes of resection arthroplasty after primary total shoulder arthroplasty (TSA), hemiarthroplasty, or reverse TSA failure.
  • To evaluate pain relief and functional recovery after resection arthroplasty.

Main Methods:

  • Retrospective multicenter review of 26 patients undergoing resection arthroplasty for failed primary shoulder implants.
  • Analysis included 6 failed TSAs, 7 failed hemiarthroplasties, and 13 failed reverse TSAs.

  • Mean follow-up was 41.8 months.
  • Main Results:

    • Significant improvement in visual analog scale pain scores post-resection arthroplasty.
    • No significant changes in mean active forward flexion or external rotation.
    • Resection after failed TSA showed better active forward flexion than after reverse TSA.

    Conclusions:

    • Resection arthroplasty effectively alleviates pain but leads to diminished postoperative function.
    • Patients undergoing resection after reverse shoulder arthroplasty failure experience worse functional outcomes.
    • No functional outcome difference observed between resection after hemiarthroplasty versus TSA.
