Children with medically refractory epilepsy show reduced default mode network (DMN) connectivity. This study investigated DMN differences in pediatric epilepsy patients compared to healthy controls, revealing significant alterations.
Area of Science:
- Neuroscience
- Pediatric Neurology
- Epilepsy Research
Background:
- The default mode network (DMN) is implicated in adult temporal lobe epilepsy.
- Limited research exists on DMN function in pediatric medically refractory epilepsy.
Purpose of the Study:
- To investigate differences in the DMN between children with medically refractory epilepsy and healthy controls.
- To assess DMN connectivity patterns in pediatric epilepsy.
Main Methods:
- Resting-state functional magnetic resonance imaging (fMRI) was conducted on 11 children with medically refractory epilepsy and 11 age-matched controls.
- Independent Component (IC) analysis identified the DMN, followed by random-effects analysis.
- Temporal correlation coefficients between key DMN regions (PCC/PCUN, mPFC, lateral parietal cortices) were calculated and correlated with clinical and cognitive parameters.
Main Results:
- Patients exhibited decreased DMN connectivity in the PCC/PCUN, bilateral lateral parietal cortex, and anterior/midcingulate regions compared to controls.
- Reduced connectivity was observed between specific DMN region pairs, including mPFC-right lateral parietal cortex and PCC/PCUN-left/right lateral parietal cortex.
- No significant correlations were found between DMN connectivity and clinical seizure parameters or IQ.
Conclusions:
- Children with medically refractory epilepsy demonstrate reduced DMN connectivity.
- Further research is warranted to explore seizure type effects on the DMN and its relationship with cognitive functions.
