Background:

  • The use of expanded criteria donors (ECD) for kidney transplantation is increasing.
  • ECD kidneys may not be suitable for single organ transplantation, leading to consideration for dual renal transplantations (DTX).

Purpose of the Study:

  • To analyze the outcomes of single transplants (STX) versus dual transplants (DTX) using kidneys from ECD older than 65 years.
  • To compare graft survival and renal function between STX and DTX recipients.

Main Methods:

  • A case-control study analyzed 80 kidneys from ECD (age > 65) between May 2007 and March 2011.
  • 40 kidneys were used for STX and 40 for DTX, with criteria based on biopsy and donor factors.
  • Outcomes including graft function, complications, and survival were compared.

Main Results:

  • DTX involved older donors and a higher proportion of women compared to STX.
  • DTX kidneys had higher biopsy scores, indicating poorer initial quality.
