Most youth with chronic daily headache (CDH) do not have comorbid psychiatric diagnoses. However, those with a psychiatric diagnosis experience greater disability and lower quality of life.
Area of Science:
- Pediatric Neurology
- Child Psychiatry
Background:
- Chronic daily headache (CDH) is a significant issue in youth.
- Comorbid psychiatric conditions are common in adult headache sufferers, but less understood in pediatric populations.
Purpose of the Study:
- To determine the prevalence of psychiatric diagnoses in adolescents with CDH.
- To investigate the association between psychiatric status and headache characteristics, disability, and quality of life in this group.
Main Methods:
- 169 youth (ages 10-17) with CDH underwent standardized psychiatric interviews (Kiddie Schedule for Affective Disorders and Schizophrenia, KSADS).
- Participants completed daily headache diaries, symptom severity scales, and measures of headache-related disability (PedMIDAS) and quality of life (PedsQL).
Main Results:
- Nearly 30% of youth with CDH had a current psychiatric diagnosis, and over 34% had a lifetime diagnosis.
- No significant link was found between psychiatric status and headache frequency, duration, or severity.
- Youth with a lifetime psychiatric diagnosis reported significantly higher functional disability and poorer quality of life.
Conclusions:
- Unlike adults, most adolescents with CDH do not present with elevated psychiatric comorbidity.
- Comorbid psychiatric diagnoses in youth with CDH are associated with increased disability and reduced quality of life.
- Findings suggest a need to address psychiatric comorbidities to improve outcomes for these patients.
