Purpose of the Study:

  • To evaluate the effectiveness of implementing the Asthma Action Plan (AAP) via telephone to enhance asthma control.
  • To determine if a phone-based strategy can improve asthma management outcomes.

Main Methods:

  • Adult asthma patients (n=48) from a university-based family medicine clinic were enrolled.
  • Initial asthma control was assessed using the ACS via phone. Patients with uncontrolled asthma (ACS <20) received medication adjustments and AAP implementation over the phone.
  • Uncontrolled patients underwent monthly phone reassessments with ongoing management adjustments until asthma control was achieved.

Main Results:

  • Out of 48 enrolled patients, 42 (87.5%) were successfully reached by phone.
  • Initially, 33 patients (69%) had controlled asthma.
  • Following phone-based AAP implementation, 7 of 9 (78%) previously uncontrolled patients achieved control, resulting in 40 (83%) patients with controlled asthma overall.

Conclusions:

  • Telephone-based asthma management utilizing the ACS and AAP is a feasible and patient-acceptable strategy.
  • This phone-based approach can significantly improve asthma control without necessitating in-office visits.
  • Telehealth interventions show promise for effective chronic disease management in primary care settings.
