Open Angle Glaucoma: Treatment01:27

Open Angle Glaucoma: Treatment

In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.

Drugs for Treatment of Crohn's Disease in IBD Using Glucocorticoids

Glucocorticoids, a class of anti-inflammatory drugs, are pivotal in treating moderate to severe Crohn's disease by inducing remission. They exhibit their anti-inflammatory action by inhibiting the production of inflammatory cytokines such as tumor necrosis factor (TNF)-α, interleukin (IL)-1, and chemokines like IL-8. In addition, they reduce the expression of inflammatory cell adhesion molecules and inhibit gene transcription of nitric oxide synthase, phospholipase A2, cyclooxygenase-2 (COX-2),...
Angle Closure Glaucoma: Treatment01:28

Angle Closure Glaucoma: Treatment

Angle-closure glaucoma, or closed-angle glaucoma, is an eye condition where the iris bulges out and blocks the iridocorneal angle, resulting in a buildup of aqueous humor and increased intraocular pressure. Immediate medical attention is necessary due to the sudden onset of symptoms. The treatment for angle-closure glaucoma includes short-term and long-term approaches. Short-term treatment involves using eye drops like pilocarpine to lower intraocular pressure by increasing aqueous humor...
Ophthalmic Drug Delivery Systems01:23

Ophthalmic Drug Delivery Systems

Ophthalmic drug delivery faces major limitations due to poor absorption across the corneal membrane. This process is primarily driven by diffusion and is influenced by two main factors: the physicochemical properties of the drug and tear drainage. Most ophthalmic drugs, such as pilocarpine, epinephrine, atropine, and local anesthetics, are weak bases. They are typically formulated at an acidic pH to enhance chemical stability. However, this leads to high ionization, reducing their ability to...

Drug Delivery: Miscellaneous Routes

Drug delivery methods like oral inhalation, nasal sprays, transdermal patches, eye drops, intravitreal injection,  and rectal administration provide localized effects with reduced toxicity.
Oral inhalation and nasal sprays swiftly transfer drugs across the respiratory epithelium's mucosal layer. Inhaled glucocorticoids and bronchodilators directly target lung conditions such as asthma, while fluticasone nasal spray mitigates allergic rhinitis.
Transdermal patches transport drugs through the...

Summary

Adding triamcinolone to viscosupplementation improved knee osteoarthritis pain and function in the first week. However, this benefit did not extend beyond week one, with no increased adverse effects observed.

Area of Science:

  • Orthopedics
  • Rheumatology
  • Pharmacology

Background:

  • Intraarticular corticosteroid injections are common for knee osteoarthritis, offering initial pain relief.
  • Viscosupplementation is an alternative treatment with a delayed onset of action.
  • Corticosteroids show greater short-term efficacy, while viscosupplementation offers longer-term pain reduction.

Purpose of the Study:

  • To determine if adding triamcinolone to viscosupplementation improves early pain and function.
  • To assess if combined therapy diminishes adverse effects compared to viscosupplementation alone.
  • To evaluate the 6-month pain and functional outcomes of combined therapy versus viscosupplementation alone.

Main Methods:

  • 104 patients with knee osteoarthritis were prospectively enrolled and randomized.
  • Group VS received hylan GF-20 (viscosupplementation) alone.
  • Group VS+T received hylan GF-20 plus triamcinolone hexacetonide.

Main Results:

  • At Week 1, the triamcinolone combination group (VS+T) showed significantly lower WOMAC and VAS pain scores.
  • No significant differences in adverse effects were observed between the groups.
  • No differences in pain or function were found at Weeks 4, 12, and 24.

Conclusions:

  • Triamcinolone hexacetonide addition enhances early (first-week) symptom and functional scores in viscosupplementation for knee osteoarthritis.
  • The benefits of adding triamcinolone do not persist beyond the first week.
  • The combination therapy does not appear to increase the incidence of adverse effects.
