Vitamin D derivatives and budesonide significantly reduce RANTES in nasal polyp fibroblasts, offering a potential complementary therapy for chronic rhinosinusitis with nasal polyps (NP). Combinations show greater efficacy than monotherapy.
Area of Science:
- Immunology
- Otolaryngology
- Pharmacology
Background:
- Eosinophils are key inflammatory cells in rhinosinusitis with nasal polyps (NP).
- RANTES is a significant chemoattractant for eosinophils in NP pathogenesis.
Purpose of the Study:
- To investigate the effect of vitamin D (VD) derivatives on RANTES expression in nasal polyp fibroblasts.
- To evaluate the potential of VD derivatives as a complementary therapy for NP.
Main Methods:
- Nasal polyp fibroblasts from 16 patients were cultured and stimulated.
- Cells were treated with calcitriol, tacalcitol, or budesonide, alone and in combination.
- RANTES levels in culture supernatants were quantified using ELISA.
Main Results:
- Calcitriol and tacalcitol significantly reduced RANTES production in a dose-dependent manner.
- Combinations of budesonide with VD derivatives were more effective than monotherapy.
- Budesonide and tacalcitol in specific ratios markedly suppressed RANTES levels.
Conclusions:
- Active vitamin D compounds downregulate RANTES production, suggesting a role in chronic rhinosinusitis with NP therapy.
- Combination therapy with budesonide and VD derivatives demonstrates superior efficacy compared to monotherapy.

Role of Skin in Vitamin D Synthesis
The solar UV B rays (290-315 nm) are absorbed by the skin, and 7-dehydrocholesterol (provitamin D3) photolyzes it to previtamin D3, which undergoes a rapid transformation to vitamin D3(cholecalciferol).
