Background:

  • Thyroid storm is a life-threatening exacerbation of hyperthyroidism.
  • Iodinated contrast media are frequently used in computed tomography (CT) examinations.

Observation:

  • A 62-year-old woman developed thyroid storm 5 hours post-CT scan with iodinated contrast (meglumine amidotrizoate).
  • Initial laboratory results showed markedly elevated serum thyroxine (T4) and free T4 levels.
  • Serum triiodothyronine (T3) and free T3 levels were within normal limits during the event.

Findings:

  • The clinical presentation and laboratory values suggest a T4-only thyroid storm.
  • This specific type of thyroid storm was potentially induced by the iodinated contrast administration.
