Background:

  • Spontaneous lens dislocation is rare, particularly without trauma or systemic disease.
  • Zonular dialysis, a weakness or tear in the ciliary zonules, can lead to lens instability.

Observation:

  • Four male patients developed spontaneous lens dislocation 0.5-6 months post-laser iridotomy.
  • Two patients had pre-existing acute primary angle closure glaucoma.
  • Dislocation varied from partial to complete into the vitreous cavity.

Findings:

  • Laser iridotomy is suspected to induce localized zonular dialysis, causing progressive weakness.
  • All affected eyes required lens removal and scleral-fixated intraocular lens implantation.
  • Excellent final visual acuity (1.2) was achieved in all cases post-surgery.

Implications:

  • Laser iridotomy may be an iatrogenic risk factor for spontaneous lens dislocation.
  • Ophthalmologists should consider this association when evaluating patients with lens instability.
  • Further research is needed to confirm the causal link and optimize management strategies.
