The Vestibular System

The vestibular system is a set of inner ear structures that provide a sense of balance and spatial orientation. This system is comprised of structures within the labyrinth of the inner ear, including the cochlea and two otolith organs—the utricle and saccule. The labyrinth also contains three semicircular canals—superior, posterior, and horizontal—that are oriented on different planes.

Summary

Elderly patients with bilateral vestibular hypofunction (BVH) exhibit higher energy consumption (EC) of the center of pressure (COP) during balance tasks. This suggests EC may be a valuable tool for assessing balance disorders in aging populations.

Background:

  • Bilateral vestibular hypofunction (BVH) affects balance in the elderly.

Purpose of the Study:

  • To evaluate the energy consumption (EC) of the center of pressure (COP) in elderly patients with BVH.
  • To determine if EC of COP is a valid parameter for balance assessment in BVH.

Main Methods:

  • Eight elderly patients with BVH and eight controls were tested on a force platform.
  • Energy consumption (EC) of the COP was calculated using discrete wavelet transform.
  • Sensory conditions included eyes open, eyes closed, and standing on foam.

Main Results:

  • BVH patients demonstrated significantly higher EC of COP compared to controls in eyes open and eyes closed conditions.
  • No BVH patients could complete the test on foam, indicating severe balance impairment.
  • Increased EC in BVH patients was noted in the 0.1-0.78 Hz frequency band when visual input was removed.

Conclusions:

  • Energy consumption (EC) of the COP is elevated in elderly individuals with BVH.
  • COP energy consumption may serve as a sensitive indicator for balance deficits in BVH patients.
  • This metric shows potential for clinical assessment of balance disorders in aging populations.
