Background:

  • Congenital abdominal wall anomalies include defects (omphalocele, gastroschisis) and conditions without defects (umbilical hernia, persistent ductus omphaloentericus, urachus).
  • Clinical presentation and surgical timing differ significantly among these conditions.

Observation:

  • A rare case of ectopic liver forming a mesodermal cyst in the umbilical region of a full-term neonate.
  • Imaging revealed a vascularized, cystic lesion without intestinal involvement; liver ultrasound was normal.
  • Surgery identified a canaliculus connecting the umbilical lesion to the liver, which was excised along with the cyst.

Findings:

  • Histological examination confirmed liver tissue with a central mesodermal cyst, connected to the liver via a bile duct.
  • The surgical approach focused exclusively on the umbilical anomaly, avoiding abdominal cavity exploration.
  • The neonate exhibited a good clinical condition six months post-operation.

Implications:

  • Ectopic liver within an omphalocele is an extremely rare condition.
  • Umbilical-focused surgical treatment for such anomalies can be sufficient.
  • This case highlights the importance of thorough histological evaluation for congenital umbilical abnormalities.
