Background:

  • Ependymoma is a primary tumor of the central nervous system.
  • Postoperative radiotherapy is a common treatment modality for ependymoma.

Purpose of the Study:

  • To evaluate failure patterns, survival rates, toxicity, and prognostic factors in patients with ependymoma treated with postoperative radiotherapy.

Main Methods:

  • Retrospective review of 30 patients with ependymoma who received surgery and postoperative radiotherapy.
  • Analysis of tumor grade (WHO II vs. III), radiation dose (median 52.8 Gy), and follow-up duration (median 51 months).

Main Results:

  • Overall survival (OS) and progression-free survival (PFS) at 5 years were 66.7% and 56.1%, respectively.
  • Tumor grade was a significant prognostic factor for OS and PFS.
  • Recurrence occurred in 46.7% of patients, with most being in-field recurrences.

Conclusions:

  • Postoperative radiotherapy for ependymoma yields favorable survival outcomes.
  • Histologic grade is a critical prognostic indicator.
  • Further investigation into intensity-modulated radiotherapy or stereotactic radiosurgery is warranted for high-risk patients, particularly anaplastic ependymoma.
