Clozapine effectively controlled aggression in adolescent boys with conduct disorder (CD) refractory to other treatments. The medication showed good tolerability with no significant side effects over 26 weeks.
Area of Science:
- Child and Adolescent Psychiatry
- Pharmacology
- Behavioral Science
Background:
- Conduct disorder (CD) in adolescents presents significant challenges, often with aggression refractory to standard treatments.
- Previous psychosocial and pharmacological interventions for severe CD aggression have shown limited efficacy in some cases.
Purpose of the Study:
- To evaluate the efficacy and tolerability of clozapine in adolescent boys with conduct disorder and severe aggression.
- To assess symptom control and safety profile of clozapine treatment over a 26-week period.
Main Methods:
- An open, naturalistic observational study involving seven adolescent boys (10-14 years) diagnosed with CD.
- Weekly monitoring of aggression levels, side effects, and hematological parameters.
- Standardized assessments (Clinical Global Impressions, Child Behavior Check List 6-18) at baseline and 26 weeks.
Main Results:
- Clozapine, at doses of 100-600 mg/day, demonstrated good tolerability with no significant adverse events or hematological changes.
- Significant reduction in aggression and overall symptom severity was observed.
- Clinical Global Impressions and Child Behavior Check List scores indicated marked symptom control.
Conclusions:
- Clozapine is a well-tolerated and effective treatment option for severe aggression in adolescent boys with conduct disorder.
- The findings suggest clozapine can provide marked symptom control in treatment-refractory cases.
- Further research into clozapine's role in pediatric behavioral disorders is warranted.
