The response to stress—be it physical or psychological, acute or chronic—involves activation of the Hypothalamic-Pituitary-Adrenal (HPA) axis. The HPA axis is part of the neuroendocrine system because it involves both neuronal and hormonal communication. Its function is to regulate homeostatic systems—metabolic, cardiovascular, and immune—providing the necessary means to respond to a stressor.

Background:

  • Functional neuroimaging offers insights into primary headache pathophysiology.
  • Previous studies identified brainstem activation in migraine and hypothalamus in cluster headache (CH).

Observation:

  • A patient with CH headache underwent functional magnetic resonance imaging (fMRI) during pain attacks.
  • fMRI scans were conducted during active CH attacks and pain-free states.

Findings:

  • Significant activation was observed in the bilateral red nucleus, ventral pons, and ipsilateral trigeminal root entry zone during CH pain.
  • The hypothalamic region also showed ipsilateral activation corresponding to the pain side.

Implications:

  • Activated brain structures are involved in motor function and reactive behaviors.
  • This activation may represent a defense or 'fight-or-flight' response during CH attacks.
