Summary

Angiotensin converting enzyme inhibitor therapy may cause hypotension during rapid infusion of stable plasma protein solution. This suggests ACE inhibitor use is a relative contraindication for rapid SPPS administration.

Area of Science:

  • Anesthesiology
  • Pharmacology
  • Cardiovascular Medicine

Background:

  • Patient received enalapril for hypertension management during surgery.
  • Surgical procedure involved groin lymph node dissection under spinal and general anesthesia.

Observation:

  • Rapid infusion of stable plasma protein solution (SPPS) led to significant hypotension and flushing.
  • The patient was on enalapril, an angiotensin converting enzyme inhibitor.

Findings:

  • A potential mechanism links SPPS infusion and ACE inhibitor pharmacology to observed adverse events.
  • Hypotension and flushing suggest an interaction between SPPS and enalapril.

Implications:

  • Angiotensin converting enzyme inhibitor (ACE inhibitor) therapy may be a relative contraindication for rapid SPPS infusion.
  • Clinical practice should consider potential interactions between ACE inhibitors and SPPS.
  • Further research is warranted to elucidate the precise mechanism of this interaction.
