  • The patient had undergone radical cystectomy and external urinary bypass 5 years prior for invasive bladder carcinoma.
  • Malignant pathology was considered controlled at the time of reconversion.

Findings:

  • The reconversion utilized a detubulized ileum for bladder replacement.
  • Renal function was preserved using an ileo-ileal intussusception mechanism.
  • The patient achieved functional micturition with both daytime and nocturnal continence, preserving external sphincter integrity.

Implications:

  • This case demonstrates the feasibility of reversing external urinary bypasses into orthotopic neobladders.
  • Reconstructive urological surgery offers potential for improved quality of life in patients with prior urinary diversions.
