Summary
Giant renal oncocytomas are rare, benign kidney tumors often found incidentally. Definitive diagnosis requires histopathology, as distinguishing them from kidney cancer pre-operatively is challenging.

Background:
- Renal oncocytoma is a benign tumor originating from distal renal tubule epithelial cells.
- These tumors are frequently asymptomatic and incidentally discovered.
- Distinguishing renal oncocytoma from renal cell carcinoma pre-operatively is difficult.
Observation:
- A case of a giant renal oncocytoma (16 x 14 cm) in a 64-year-old male is presented.
- The patient was asymptomatic and the tumor was pre-operatively diagnosed as a malignant kidney tumor.
- Radical nephrectomy was performed, revealing a tumor with a central hyaline scar.
Findings:
- Microscopic examination showed uniform polygonal cells with eosinophilic cytoplasm.
- Immunohistochemistry revealed tumor cell reactivity to CK AE1/AE3 and CD 117.
- Tumor cells were negative for CK 7, RCC marker, and Vimentin.
Implications:
- Giant renal oncocytomas are rare and have a benign clinical course.
- Clinical differentiation from malignant kidney tumors is not feasible.
- Histopathological examination with immunohistochemistry is essential for definitive diagnosis.
