Purpose of the Study:

  • To review the experience with leaflet extension in rheumatic mitral valve repair.
  • To evaluate the mid-term clinical and echocardiographic outcomes of this technique.

Main Methods:

  • Leaflet extension with glutaraldehyde-treated autologous pericardium was performed in 62 rheumatic patients between 2003 and 2010.
  • Patients underwent various additional repair procedures, including neo-chordal replacement and commissurotomy, with annuloplasty rings used for stabilization.

Main Results:

  • No mortality was observed. At a mean follow-up of 36.5 months, 64.5% of patients had no/trivial mitral regurgitation.
  • Mild MR was present in 22.6%, and moderate to severe MR in 11.7%.
  • Estimated freedom from reoperation and valve failure at 5 years was 96.8% and 91.6%, respectively.

Conclusions:

  • Mitral valve repair with leaflet extension in rheumatic disease yields favorable early and mid-term outcomes.
  • Increased utilization of this technique can improve the success and longevity of repair for complex rheumatic mitral valve disease.
