Summary

A rare cross-limb forearm replantation in a woman with bilateral arm amputation restored significant function. This procedure offered superior independence compared to a prosthesis, highlighting its potential in similar complex cases.

Background:

Bilateral arm amputation presents significant challenges for functional restoration.
Orthotopic replantation is not always feasible in severe traumatic amputations.

Observation:

A 46-year-old woman with traumatic bilateral arm amputation underwent a left forearm replantation to the right proximal forearm.
Two-year follow-up demonstrated a sensate hand with functional recovery of flexors and extensors.

Findings:

The patient achieved a high level of function, outperforming her contralateral prosthetic limb.
The DASH score was 93.9 and Chen's score was III, indicating medium functional recovery.
