Background:

  • Induced sputum using hypertonic saline is a potential alternative to bronchoalveolar lavage for interstitial lung diseases (ILD).
  • This method is suggested to be safer and more cost-effective for ILD patient evaluation.

Purpose of the Study:

  • To assess the safety and feasibility of sputum induction in patients with ILD.
  • To compare the cellular profiles obtained from induced sputum with results from bronchoalveolar lavage (BALF).

Main Methods:

  • Twenty patients with ILD underwent sputum induction with 4.5% saline within two weeks of bronchoalveolar lavage.
  • Cellular counts (total and differential) and viability were analyzed.
  • Statistical analysis included Wilcoxon test and Spearman's rank correlation (p<0.05 significance).

Main Results:

  • A satisfactory sputum sample was obtained in 75% of subjects (15 out of 20).
  • Sputum induction was discontinued in one patient due to a significant drop in peak expiratory flow (PEF).
  • Cellular profiles differed significantly between induced sputum and BALF (P < .05), except for eosinophils, with no significant correlation found between the methods.

Conclusions:

  • Induced sputum is a safe and feasible diagnostic tool for interstitial lung diseases.
  • Despite differing cell counts compared to bronchoalveolar lavage fluid, sputum cellular profiles show potential in differentiating between various ILD subtypes.
