Peptic Ulcer Disease III: Clinical Manifestations and Diagnostic Studies01:28

Peptic Ulcer Disease III: Clinical Manifestations and Diagnostic Studies

Peptic ulcer disease (PUD) presents with diverse symptoms depending on the location and severity of the ulcer. Clinical manifestations of peptic ulcer include dull pain and a burning sensation in the mid-epigastric region.
Few clinical manifestations differentiate gastric ulcers from duodenal ulcers. Distinctions in the location, timing, and pain relief are crucial for healthcare providers in differentiating between gastric and duodenal ulcers during clinical assessments.

Endoscopic Procedures I: Esophagogastroduodenoscopy

An Esophagogastroduodenoscopy (EGD) is a diagnostic procedure in which an endoscopist uses a flexible, lighted endoscope to visualize the upper gastrointestinal (GI) tract. The procedure includes visualizing the oropharynx, esophagus, stomach, and the first part of the small intestine, the duodenum.
During an EGD, the endoscope can be used to:

Barrett Esophagus-II: Clinical Manifestations and Management

Individuals with Barrett's esophagus are often asymptomatic, but they may experience symptoms commonly associated with GERD, such as heartburn and acid regurgitation. Additional symptoms can include difficulty swallowing, chest pain, unintentional weight loss, blood in the stool (which may appear black, tarry, or bloody), and episodes of vomiting.
To diagnose Barrett's esophagus, healthcare providers often recommend an endoscopy for those showing symptoms of acid reflux. The procedure entails...

Endoscopic Studies I: Bronchoscopy and Thoracoscopy

Endoscopy is a non-surgical medical technique used to examine a person's internal organs and vessels. This lesson will focus on two types of endoscopic studies: bronchoscopy and thoracoscopy.
Bronchoscopy
Description
Bronchoscopy is a procedure that involves direct visualization of the larynx, trachea, and bronchi for diagnostic and therapeutic purposes. A flexible fiber optic or rigid bronchoscope is used to carry out the procedure. The fiber-optic bronchoscope is more frequently used due to...

Summary

The PedsQL EoE Module effectively measures health-related quality of life in children with eosinophilic esophagitis (EoE). It shows that active disease and dietary restrictions negatively impact quality of life for pediatric EoE patients.

Area of Science:

  • Pediatric Gastroenterology
  • Immunology
  • Quality of Life Research

Background:

  • Eosinophilic esophagitis (EoE) is a chronic allergic inflammatory disease of the esophagus.
  • Limited data exists on health-related quality of life (HRQOL) in pediatric EoE patients.
  • The PedsQL EoE Module is a tool designed to assess HRQOL in this population.

Purpose of the Study:

  • To evaluate the measurement properties of the PedsQL EoE Module.
  • To assess the reliability and validity of the PedsQL EoE Module in a pediatric cohort.
  • To understand how HRQOL is affected by disease activity and treatment in pediatric EoE.

Main Methods:

  • A multisite study involving 196 pediatric patients diagnosed with EoE and 262 parents.
  • Administration of the PedsQL EoE Module and PedsQL Generic Core Scales.
  • Analysis of feasibility, reliability (internal consistency, test-retest), and validity (group comparisons).

Main Results:

  • The PedsQL EoE Module demonstrated excellent feasibility, reliability, and scaling success.
  • Scores were significantly lower in patients with active histologic disease compared to those in remission.
  • Patients undergoing dietary restrictions reported worse HRQOL compared to those without restrictions.

Conclusions:

  • The PedsQL EoE Module exhibits excellent measurement properties for assessing HRQOL in pediatric EoE.
  • Active disease and dietary interventions are associated with diminished HRQOL in children with EoE.
  • The module is suitable for evaluating disease-specific HRQOL in clinical research and practice for pediatric EoE.
