Does inhibition of proteolytic activity improve adhesive luting?

Summary
Matrix metalloproteinase (MMP) inhibitors did not improve the long-term bond durability of self-adhesive composite cements to dentin. Testing showed no significant effect of MMP inhibitors on microtensile bond strength after aging.

Background:
- Endogenous enzymes, specifically matrix metalloproteinases (MMPs), are implicated in the degradation of adhesive restoration-tooth interfaces.
- MMP inhibitors have shown potential in slowing down bond degradation, but their efficacy with composite cements is less understood.
Purpose of the Study:
- To investigate the impact of two MMP inhibitors, chlorhexidine digluconate (CHX) and galardin, on the microtensile bond strength (μTBS) of self-adhesive composite cements bonded to dentin.
- To assess whether MMP inhibitors can enhance the durability of the bond between composite cements and dentin over time.
Main Methods:
- Two self-adhesive composite cements (RelyX Unicem 2, Clearfil SA) and one etch-and-rinse control (Nexus 3) were used.
- Dentin surfaces were pretreated with either 2% CHX or 0.2 mM galardin, or left untreated.
- Microtensile bond strength (μTBS) was measured immediately after cementation and after 6 months of water storage.
Main Results:
- No significant effect of MMP inhibitors (CHX or galardin) on the μTBS was observed.
- A significant decrease in μTBS was found for all tested cements after 6 months of aging.
- The only exception was the etch-and-rinse cement used without MMP inhibitors, which maintained its bond strength.
Conclusions:
- MMP inhibitors did not prevent the age-related decrease in μTBS for the tested self-adhesive composite cements.
- The use of MMP inhibitors did not improve the long-term luting durability of these composite cements.
- Further research may be needed to explore alternative strategies for enhancing the longevity of adhesive restorations.
