Background:

  • Traditionally, elective hernia operations provided essential training for surgical residents.
  • Daycase surgical units (DCUs) are increasingly performing procedures previously done in general elective settings.
  • This study investigates the potential of DCUs to enhance surgical training in hernia repair.

Purpose of the Study:

  • To assess the effectiveness of Daycase Surgical Units (DCUs) in improving surgical training for hernia operations.
  • To compare the performance of surgical trainees leading hernia operations in DCUs versus consultants.

Main Methods:

  • Retrospective analysis of Operative Room Information System (ORMIS) data from January 2007 to 2012.
  • Data included surgeon, procedure type, procedure time (PT), length of stay (LOS), and 30-day readmissions and deaths.
  • Hernia operations performed in DCU versus traditional settings were compared.

Main Results:

  • Out of 4668 hernia operations, 3063 were performed in DCUs.
  • Trainees assisted in 24.6% and led 7.8% of cases.
  • No significant difference in PT, LOS, readmissions, or 30-day mortality was found between trainees and consultants for many hernia repair types in DCUs.

Conclusions:

  • DCUs represent an underutilized resource for surgical trainees to gain experience in hernia operations.
  • Trainees leading hernia operations in DCUs achieve comparable procedure times and complication rates to consultants.
  • Encouraging trainee participation in DCU hernia cases under supervision is crucial for competency development.
