Pigmented villonodular synovitis (PVNS) most often affects middle-aged women, commonly involving the knee. Surgical treatment is effective but recurrence rates are high.

- Pigmented villonodular synovitis (PVNS) is a rare synovial joint lesion with unclear etiology.
- Limited information exists on PVNS clinical history, diagnosis, treatment, and long-term outcomes.
Observation:
- A study analyzed 75 patients (81 joints) with PVNS, treated with synovectomy +/- arthroplasty.
- PVNS predominantly affected middle-aged women, with the knee being the most common site, followed by the hip and ankle.
- Trauma history was noted in 42 patients; pain and swelling were primary symptoms.
Findings:
- Pathological examination confirmed PVNS in all cases.
- Recurrence was observed in 12 patients, with four experiencing multiple recurrences.
- Five patients developed PVNS post-arthroplasty.
Implications:
- Surgical excision, with or without arthroplasty, is an effective PVNS treatment.
- High recurrence rates necessitate ongoing monitoring and potential re-intervention.
- Further research into PVNS etiology is warranted to improve management strategies.
