Summary

Contact hypersensitivity is common in chronic otitis externa, particularly in older adults. Neomycin was the most frequent cause of allergic reactions in this patient group.

Area of Science:

  • Dermatology
  • Otolaryngology

Background:

  • Chronic otitis externa (COE) is an inflammatory condition of the external ear canal.
  • Contact hypersensitivity is a potential contributing factor to chronic inflammatory skin conditions.

Purpose of the Study:

  • To investigate the prevalence of contact hypersensitivity in patients with chronic otitis externa.
  • To identify common allergens responsible for hypersensitivity reactions in this population.

Main Methods:

  • Standardized patch testing was performed on 37 patients with chronic otitis externa, evaluating reactions to 49 common agents.
  • Patient demographics, including age and duration of otitis externa, were recorded and analyzed in relation to patch test results.

Main Results:

Related Experiment Videos

  • A high incidence of positive patch test reactions (58%) was observed, exceeding previous study findings.
  • Older patients (average age 56.7 years) were significantly more likely to exhibit hypersensitivity compared to younger patients (average age 34.9 years).
  • Neomycin was identified as the most frequent sensitizing agent, responsible for 32% of positive reactions.
  • Conclusions:

    • Contact hypersensitivity is a significant factor in chronic otitis externa, especially in older individuals.
    • The findings suggest that contact hypersensitivity may not be a primary etiological factor in younger patients with COE.
    • Neomycin allergy should be considered in the management of chronic otitis externa.
