Background:

  • Frontal lobe atrophy (FLA) is associated with late-life depression.
  • The role of FLA in depression after stroke (DAS) is not well understood.

Purpose of the Study:

  • To investigate the association between FLA and the development of DAS.

Main Methods:

  • A cohort of 705 Chinese patients with acute ischemic stroke was assessed.
  • Depression after stroke (DAS) was diagnosed using the Structural Clinical Interview for DSM-IV three months post-stroke.
  • Frontal lobe atrophy (FLA) was evaluated.

Main Results:

  • 12.1% of patients developed DAS.
  • Severe FLA was more prevalent in patients with DAS (14.1% vs 5.6%).
  • Severe FLA was an independent predictor of DAS (OR 2.6, 95% CI 1.2-5.9).

Conclusions:

  • Frontal lobe atrophy (FLA) may contribute to the pathogenesis of depression after stroke (DAS).
  • These findings support the hypothesis that cumulative vascular burden is important in predicting DAS.
  • Further research is needed to understand FLA's impact on DAS clinical presentation, treatment, and outcomes.
