Summary

An unopened loop ileostomy, an alternative to conventional ileostomies for protecting colorectal anastomoses, was evaluated. This small study found it associated with significant morbidity and does not support its routine use.

Background:

  • Defunctioning loop ileostomies protect distal colorectal anastomoses.
  • Conventional ileostomy creation and closure can lead to significant morbidity.
  • An alternative approach using an unopened loop ileostomy was investigated.

Purpose of the Study:

  • To evaluate the efficacy and safety of an unopened loop ileostomy.
  • To compare its complication rate and closure ease with conventional ileostomies.

Main Methods:

  • Patients undergoing colorectal resection with distal anastomosis received an unopened loop ileostomy.
  • Complications and ease of closure were prospectively recorded.

Main Results:

  • Seven out of 12 patients (58%) experienced complications.
  • Five patients had difficulty with ileal loop replacement.
  • One patient developed a wound abscess and pseudomembranous colitis.

Conclusions:

  • The unopened loop ileostomy was associated with a high complication rate in this small cohort.
  • The study does not support the routine use of unopened loop ileostomies for high-risk colorectal anastomoses.
