This study found that a 3D digital templating method reliably estimates tibial coverage during total knee replacement. The method showed excellent consistency between observers and across different prosthesis designs.

Area of Science:

  • Orthopedic Surgery
  • Biomedical Engineering
  • Radiology

Background:

  • Accurate estimation of tibial coverage is crucial for successful total knee replacement outcomes.
  • Current methods for assessing tibial coverage lack sufficient reliability data.
  • Three-dimensional (3D) digital templating offers a potential solution for precise coverage assessment.

Purpose of the Study:

  • To evaluate the intra- and interobserver reliability of a 3D digital templating method for estimating tibial coverage in total knee replacement.
  • To compare the reliability of this method using both symmetric and asymmetric prosthesis designs.
  • To identify factors influencing the reliability of tibial coverage measurement.

Main Methods:

  • 120 template procedures were conducted by three observers using specific rotational and overhang criteria.

  • Total and sub-region tibial coverage were calculated using a 3D digital templating technique.
  • Intra- and interobserver reliability was assessed using intraclass correlation coefficients (ICC).
  • Main Results:

    • Excellent intra- and interobserver reliability was achieved for total tibial coverage estimation.
    • Reliability was high even with minimal component overhang (ICC = 0.87) or no overhang (ICC = 0.92).
    • Rotational restrictions had a minimal influence on the overall reliability of the method.

    Conclusions:

    • The described 3D digital templating method provides a reliable approach for measuring tibial coverage in total knee replacement.
    • This technique can be consistently applied by different observers, enhancing surgical planning.
    • While rotational axis selection has a minor effect, the method's overall reliability is robust.
