Purpose of the Study:

  • To evaluate the necessity and clinical utility of prophylactic postoperative antibiotics in patients undergoing enucleation or ocular evisceration.
  • To determine if withholding postoperative antibiotics impacts infection rates in these procedures.

Main Methods:

  • Retrospective, multicenter comparative case series.
  • Analysis of 644 patient records from 1996-2011 across four institutions.
  • Comparison of postoperative infection rates between patients receiving and not receiving postoperative antibiotics.

Main Results:

  • No statistically significant difference in postoperative infection rates (orbital cellulitis) was observed between groups (p=0.52).
  • Two suspected infections occurred, one in each group; no culture-positive infections were identified.
  • Patients with infectious indications for surgery did not show increased infection risk when not receiving postoperative antibiotics (p=0.79).

Conclusions:

  • Withholding prophylactic postoperative antibiotics is clinically safe in sterile enucleation and evisceration surgeries, even with alloplastic implants.
  • Adherence to Centers for Disease Control and Prevention guidelines for antibiotic cessation within 24 hours is supported.
  • Results should not be extrapolated to non-sterile surgical procedures.
