Mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes (MELAS) can cause stroke-like episodes. Proton magnetic resonance spectroscopy (1H-MRS) detected elevated lactate in a child with MELAS, despite normal MRI scans, indicating a mild, transient event.
