Background:

Physician practice environments are evolving.
Understanding surgeon employment trends is crucial for healthcare planning.
The shift from independent practice to employment has been observed across specialties.

Purpose of the Study:

To analyze surgeon employment trends in the United States from 2001 to 2009.
To characterize the changing practice environments chosen by surgeons.
To identify demographic factors associated with employment preferences.

Main Methods:

Analysis of American Medical Association Physician Masterfile data (2001-2009).
Surgeons defined using AMA specialty and ABMS certification data.
Inclusion criteria: active, nonfederal, non-resident physicians <80 years old.

Main Results:

Self-employed surgeons decreased from 48% to 33% (2001-2009).
68% of US surgeons are now employed.
A 32% increase in full-time hospital employment (2006-2011) was observed, especially among younger and female surgeons.

Conclusions:

General surgeons and subspecialists are increasingly opting for hospital employment over independent practice.
Younger and female surgeons show a stronger preference for employment, particularly in large group practices.
This represents a significant paradigm shift in the surgical profession.
